FMGE (Foreign Medical Graduate Examination) is a screening test mandated by the National Medical Commission (NMC) for Indian citizens and Overseas Citizens of India (OCI) who have obtained an MBBS degree from a foreign medical university. Only after passing FMGE can these graduates obtain provisional or permanent registration from NMC to practice medicine in India. The exam covers the entire MBBS curriculum and is designed to ensure that foreign medical graduates meet the same competency standards as Indian MBBS graduates. The examination is conducted by NBEMS twice a year — in June and December.

The FMGE pass rate has historically been between 15% and 25%, making it one of the most challenging licensing examinations in India despite having no negative marking. The low pass rate is attributed to several factors: inadequate preparation, language barriers (many candidates studied in Russian, Chinese, or Ukrainian-medium programs), different medical education standards abroad, insufficient clinical exposure in some foreign universities, and the comprehensive 300-question format covering all 19 subjects. Candidates who prepare systematically with a complete MBBS subject revision and dedicated FMGE question bank practice consistently achieve better results.

A 6-month FMGE preparation strategy should begin with a complete subject-wise revision covering all 19 MBBS subjects in the first 3-4 months. Prioritize high-weightage subjects: Medicine (30-40 questions), Surgery (25-30 questions), OBG (20-25 questions), Pediatrics (20-25 questions), Pharmacology (20-25 questions), and Pathology (20 questions). Use a quality FMGE question bank (Marrow, Prepladder, Dams Study Material) for daily practice. Solve at least 10 full-length FMGE mock tests (300 questions each) in the final 6 weeks. Focus on clinical application questions rather than purely factual recall.

After passing FMGE, a candidate applies for provisional or permanent registration from NMC (National Medical Commission) and the State Medical Council. With this registration, they can practice medicine in India, join government or private hospitals, pursue postgraduate admissions (through NEET PG, AIIMS PG, JIPMER PG, DNB CET), or start clinical practice. Internship completion (from the foreign university or through a bridging internship in India) is typically required before permanent registration. FMGE-qualified doctors can also appear for UPSC MBBS-based posts (civil services requiring medical degree) with valid NMC registration.

FMGE is applicable to MBBS degrees from WHO-listed foreign medical universities only. Countries from which large numbers of Indian students pursue MBBS include Russia, Ukraine, China, Bangladesh, Nepal, Philippines, Kyrgyzstan, Kazakhstan, Georgia, Armenia, and Caribbean nations. The university must be listed in both the WHO World Directory of Medical Schools (WDOMS) and the NMC's published list of recognized foreign institutions. Degrees from countries or universities not on the NMC-recognized list are not eligible for FMGE. Candidates should verify university recognition before enrolling in any foreign medical program.
