Government pharmacists in state health departments are posted at district hospitals, PHCs, CHCs, and government medical stores. Their responsibilities include dispensing prescribed medications accurately, maintaining drug inventory and stock records, procuring medicines under the Essential Medicines List (EML), checking expiry dates and storage conditions, counseling patients on medication use and side effects, maintaining narcotic drug registers under NDPS Act compliance, preparing extemporaneous preparations when needed, and participating in National Disease Control Program drug distribution (TB, malaria, leprosy, HIV antiretrovirals).

State Pharmacist recruitment exams typically test three core areas: Pharmacy (pharmaceutics, pharmaceutical chemistry, pharmaceutical jurisprudence, hospital pharmacy), Pharmacology (drug classifications, mechanisms of action, adverse effects, drug interactions), and Therapeutics (disease management, treatment protocols, rational drug use, essential medicines). The difficulty is rated moderate, accessible to well-prepared D.Pharm or B.Pharm holders. Some states also include General Studies and their state-specific GK section. Knowing the Essential Medicines List and National List of Essential Medicines (NLEM) is critical for government pharmacy exams.

Most state government pharmacist posts at PHC and CHC level accept D.Pharm (Diploma in Pharmacy, 2 years) as the minimum qualification. B.Pharm (Bachelor of Pharmacy, 4 years) holders can also apply and may get preference for senior pharmacist posts or central government positions. State pharmacy examination boards (like UP SSSC, Rajasthan SSC) typically have separate notifications for D.Pharm and B.Pharm holders with corresponding pay levels. B.Pharm holders eligible for GPAT (Graduate Pharmacy Aptitude Test) may also pursue hospital pharmacy specialization for higher-level hospital pharmacist positions.

State government pharmacists are typically placed in Pay Level 5–6 under 7th Pay Commission (basic pay ₹29,200–₹1,12,400). With state-specific DA, HRA, and other allowances, the in-hand salary for a state government pharmacist generally ranges from ₹35,000 to ₹55,000 per month depending on state and posting location. Central government pharmacist posts (AIIMS, ESIC, Railways, CGHS) offer slightly higher pay under central pay scales. NHM contractual pharmacist positions offer lower consolidated honorariums (₹12,000–₹18,000/month) without full benefits.

For state pharmacist exams, the highest-priority subjects are Pharmacology (especially classification of drugs, mechanisms, side effects, and drug interactions—this forms the bulk of questions), Pharmaceutical Jurisprudence (Drugs and Cosmetics Act, NDPS Act, Pharmacy Act, Drugs Prices Control Order—essential for legal compliance questions), Hospital Pharmacy (drug distribution systems, unit dose dispensing, ADR reporting, formulary management), and Pharmaceutics (dosage forms, bioavailability, pharmacokinetics). Therapeutics questions on commonly prescribed conditions (hypertension, diabetes, TB, malaria, infections) should also be thoroughly prepared.
